Proj. no. 55-05, Sub Proj. no.5; no.1 (Final report) (CONFIDENTIAL)ข้อมูลการพิมพ์: Pathumthani : Thailand Institute of Scientific and Technological Research, 2019 รายละเอียดตัวเล่ม: 79 p. 30 cm.ชื่อเรื่องอื่นๆ: โครงการวิจัยเรื่อง : วิจัยและพัฒนาผลิตภัณฑ์สำหรับบรรเทาอาการจากไมเกรนด้วยสมุนไพรสกุลเบญจมาศ ผลิตภัณฑ์และเทคโนโลยี : แหล่งของดอกเก็กฮวยขาว องสะเมิง จ.เชียงใหม่สาร Marker = Luteolinทำผลิตภัณฑ์ยาชง (แกรนูชง) การนำไปต่อยอด : ผลิตภัณฑ์เสริมอาหารสำหรับบรรเทาอาการจากไมเกรนด้วยสมุนไพรสกุลเบญจมาศ/บริษัทเอกชนผู้ผลิตยาและผลิตภัณฑ์เสริมสุขภาพจากสมุนไพร จุดเด่นของเทคโนโลยี/ ผลิตภัณฑ์ : สมุนไพรจากดอกเก๊กฮวยขาว - ให้ผลในการบรรเทาอาการปวดศีรษะจากไมเกรน - ผงชงละลายน้ำ, มีกลิ่นหอม, รับประทานง่ายหัวเรื่อง: โรคไมเกรนสาระสังเขป: การวิจัยในครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาผลิตภัณฑ์ในการบรรเทาอาการปวดศีรษะจากไมเกรน ด้วยพืชสกุลเบญจมาศให้ได้เป็นผลิตภัณฑ์ในรูปแบบยาชงละลายน้ำ, มีกลิ่นหอม รับประทานง่ายและมี ประสิทธิภาพในการบรรเทาอาการปวดไมเกรน โดยนำดอกเก๊กฮวยขาวมาสกัด พบว่า มีสาร luteolin เป็นสารสำคัญ จึงได้นำเอาสารสกัดดอกเก๊กฮวยมาศึกษาฤทธิ์ทางเภสัชวิทยาในการบรรเทาอาการปวด จากไมเกรนในหนูขาว พบว่า สารสกัดดอกเก๊กฮวย ขนาด 500 มิลลิกรัมต่อกิโลกรัม สามารถบรรเทา อาการเจ็บปวดได้อย่างมีนัยสำคัญทางสถิติเช่นเดียวกับยาแก้ปวดมาตรฐาน paracetamol และ Ibuprofen จึงได้นำเอาสารสกัดดอกเก๊กฮวยมาพัฒนาเป็นผลิตภัณฑ์โดยให้ชื่อว่า "4Graine" แล้วนำมา ศึกษาฤทธิ์ทางเภสัชวิทยาของผลิตภัณฑ์ในขั้นต่อไป พบว่า "4Graine" สามารถลดความเจ็บปวดที่รุนแรง ในหนูขาวที่เหนี่ยวนำด้วยวิธีserotonin-evoked thermal hyperalgesia ได้อย่างมีนัยสำคัญ (p<0.05) โดยสามารถลดระดับ serotonin (5-HT) ใน serum ได้เช่นเดียวกับยามาตรฐาน sumatriptan และยังมี ฤทธิ์ในการต้านการอักเสบในหนูขาวที่เหนี่ยวนำด้วยการฉีดคารีจีแนน ที่อุ้งเท้าหนูได้อย่างมีนัยสำคัญ เช่นเดียวกับสารมาตรฐาน phenylbutazone เมื่อศึกษาถึงกลไกการออกฤทธิ์ พบว่า ผลิตภัณฑ์ "4Graine" สามารถเพิ่มระดับ 5-HT ในสมองหนูขาวที่ถูกเหนี่ยวนำให้มีระดับลดลงด้วย reserpine ได้ อย่างมีนัยสำคัญทางสถิติซึ่งผลที่ได้นี้เป็นไปในทางเดียวกับยามาตรฐานในการรักษาอาการปวดศีรษะ ไมเกรน sumatriptan จึงเป็นไปได้ว่า "4Graine" จะออกฤทธิ์เป็น "serotonin agonist" เช่นเดียวกันกับ sumatriptan เมื่อนำมาประเมินความปลอดภัยในสัตว์ทดลอง พบว่า ผลิตภัณฑ์ "4Graine" มีค่า LD50>6,000 มิลลิกรัมต่อกิโลกรัม และเมื่อให้ผลิตภัณฑ์ต่อเนื่องกันนาน 90 วัน เพื่อศึกษาความเป็นพิษ กึ่งเรื้อรัง พบว่า ไม่ก่อให้เกิดความผิดปกติอย่างมีนัยสำคัญทั้งในส่วนของน้ำหนักตัว อัตราการกินอาหาร ค่าทางโลหิตวิทยา ค่าทางเคมีคลินิกในเลือดและปัสสาวะ รวมถึงพยาธิสภาพของอวัยวะหลักต่างๆ การศึกษาความคงตัวของผลิตภัณฑ์ "4Graine" พบว่า สามารถเก็บผลิตภัณฑ์ที่อุณหภูมิ30 องศาเซลเซียส ได้นานเกิน 6 เดือน โดยที่ปริมาณ luteolin ลดลงไม่เกิน 10 เปอร์เซ็นต์และยังพบว่า ไม่มีการ ปนเปื้อนของทั้งเชื้อก่อโรคและโลหะหนักในผลิตภัณฑ์ การศึกษาขั้นคลินิกในส่วนของความปลอดภัย และประสิทธิผลต่อการบรรเทาอาการปวดศีรษะจากไมเกรนในอาสาสมัครเปรียบเทียบกับยาหลอก พบว่า "4Graine" สามารถยับยั้งอาการปวดไมเกรนได้52 เปอร์เซ็นต์โดยอาสาสมัครมีความพึงพอใจในการใช้ ผลิตภัณฑ์"4Graine" เพื่อรักษาอาการไมเกรน 57.1 เปอร์เซ็นต์และไม่พบผลข้างเคียงใดๆ จากการใช้ ผลิตภัณฑ์นี้. แต่อย่างไรก็ดีการเพิ่มจำนวนอาสาสมัครจะเป็นการยืนยันประสิทธิผลและความปลอดภัย ให้ชัดเจนยิ่งขึ้นสาระสังเขป: The purpose of this study aims to research and develop a natural product for relieving migraine headaches using plants from chrysanthemum genus in a form of infusion which is water soluble, offers a simple fragrance, and provides effectiveness in alleviating migraine. Chrysanthemum indicum L. (chrysanthemum) found to contain luteolin, which is a main chemical compound in the flower of chrysanthemum. The extract of white chrysanthemum flower was selected to study in pharmacological effects for pain relieving from migraine in rats. The studies showed that 500 mg/kg chrysanthemum significantly possessed analgesic activity in rats as well as paracetamol and ibuprofen, standard painkillers, did. Hence, the chrysanthemum extract was formulated in an infusion product, and was named as "4Graine". This infusion product was used for the rests of this study. The pharmacological study exhibited that "4Graine" markedly diminished severe pains in serotonin-evoked thermal hyperalgesia rats by minimizing the levels of serotonin (5-HT) in serum similar to sumatriptan. Moreover, "4Graine" definitely showed anti-inflammatory effect in carrageenan-induced rat paw edema corresponding to phenylbutazone. To study the mechanism of actions of "4Graine", decreasing the level of 5-HT in rat's brain was performed by reserpine injection. The result revealed that "4Graine" statistically significant boosted the levels of the brain 5-HT in reserpine rats. An increasing in the brain 5-HT was also found in sumatriptan rats, a standard drug for the migraine treatment in the 5-HT agonist group. Therefore, it was possible that "4Graine" acted as "5-HT agonist" similarly to sumatriptan. The safety evaluation in rats expressed that the LD50 of "4Graine" was greater than 6,000 mg/kg, and subchronic toxicity for 90-day oral administration displayed no abnormal significance in terms of weight, feeding rate, hematology, the clinical chemistry in blood and urine, and the pathology of major organs. The amount of luteolin in "4Graine" was stabilized for longer than 6 months in 30C. It was not exceeding 10% reduction. For quality control of the product, there was no contamination of pathogens and heavy metals in "4Graine". In clinical studies of safety and effectiveness for relieving migraine headaches in volunteers compared with placebo, "4Graine" was found to inhibit migraine pain for 52%. Volunteers satisfied using "4Graine" to treat migraine for 57.1%. No any side effects from the use of this product were found. However, increasing the number of subjects to confirm the efficacy and safety of the product more understandably should be performed in the further study.
